The Illinois School Code provides for a continuation of learning when a student may need to be educated temporarily away from the school building due to a medical condition. The purpose of Home/Hospital Services is to facilitate that learning through a certified teacher.

Home/Hospital Services may be provided to any student when the student will be away from the school building for a minimum of 2 or more consecutive weeks of school or ongoing intermittent absences totaling 10 or more school days due to the identified medical condition as confirmed by a licensed physician. (34 CFR 300.39 and 300.115 and Section 14-13.01 of the school code [105 ILCS 5/14-13.01(a)] and ISBE Rule 226.300).

Home/Hospital services require:

> Completed home/hospital forms including a signed physician’s certification indicating:

  • Identification of a specific diagnosis
  • Recommendation of services including limits/restrictions
  • Anticipated duration of services

> Parental consent

During student absence from school, a certified teacher will serve as a liaison between the school and home/hospital. The classroom teacher(s) continues to be responsible for class content and will assist in providing needed information/materials.
